Accession Exam Completed Date/Time MR10454674 MRI BREAST BILATERAL W AND W/O CONTRAST 05/09/2024 12:01 Requesting: PATEL, VAISHALI M ---------------------------------------------------------------------------- HISTORY: 48-year-old female with a history of right breast cancer status post mastectomy and reconstruction with a silicone implant in July 2023. Patient had a benign MRI-guided biopsy in the left breast at 2:30 which demonstrated benign breast tissue with mild stromal fibrosis. Family history of breast cancer in paternal cousin. History of oral contraceptive use for 11 years. Patient presents for screening breast MRI. COMPARISON: Screening mammogram dated 11/17/2023. Breast MRI dated 5/5/2023. TECHNIQUE: Multisequence MRI of both breasts was performed on a 3 Tesla magnet both before and after administration of 13 mL intravenous gadolinium. Five dynamic enhanced phase sequences were obtained. FINDINGS: Heterogeneously dense tissue is present. Following intravenous gadolinium administration there is moderate background parenchymal enhancement. RIGHT BREAST: There are postsurgical changes of right mastectomy and silicone implant reconstruction. The implant appears intact. There is no significant peri-implant fluid collection. There is mild thin enhancement along the implant capsule, likely postsurgical change. There is an no suspicious mass or suspicious non-mass enhancement in the residual right breast tissue. There are no skin or nipple abnormalities. LEFT BREAST: There is stable linear non-mass enhancement in the left breast at 2:30 with a biopsy clip along the anterior margin. This is biopsy-proven benign breast tissue. There are no new suspicious masses or areas of suspicious non-mass enhancement in the left breast. There are no skin or nipple abnormalities. LYMPH NODES: There are no pathologic axillary or intramammary lymph nodes. IMPRESSION: 1. Expected postoperative changes of right mastectomy and implant reconstruction. 2. No evidence of breast malignancy. 3. No pathologic adenopathy. RECOMMENDATION: Given the patient's elevated risk of breast cancer, recommend continuing to alternate annual screening mammography and annual screening breast MRI every 6 months.
