Yesterday's assessment went well and our dad has been approved to transfer to his next placement. Unfortunately, due to some staffing shortages, it won't be this week. BUT his discharge from the hospital is planned for first thing Monday morning! Our dad was a little upset with the news that he won't be leaving the hospital this week, but is glad we have a date and time for discharge.
He'll be moving directly to a memory care facility in Hutchinson, MN as we work to better understand what his baseline cognition will be. His delirium is improving, but he is still disoriented every morning and continues to have gaps in memory, especially of the last 5+ years.
The community he is moving to is small and there are a few people his age, so that will be nice for him. Although it will be a secure facility due to a safety risk of wandering, he'll have a lot more independence than he has had at the hospital and we'll be able to take him places (shopping, out to eat, the lake, etc.). Brianna, Jordan, and their families live in the same town so he'll be able to connect with them a lot more as well.
