While at work, I received a call from Dr. Nair. She updated me that after consulting with UAB pediatric cardiology (apparently seven other UAB pediatric cardiologists reviewed the echo), she was confident about the following diagnoses, in addition to the already determined dextrocardia and VSD: DORV and TGA. She also said that she was now thinking Archie had a “mild to moderate” right ventricle hypoplasia and may not have to have a single ventricle repair. Hallelujah for that possibility! He may instead be a candidate for other repairs, possibly an arterial switch operation OR if his pulmonary valve isn’t adequate, something called a Rastelli procedure, in which a conduit is placed between his right ventricle and pulmonary artery. Both of these are open heart surgeries but the prognosis is good. For the Rastelli, another surgery would probably be required later in life, as Archie outgrows the conduit. Still, either of these surgeries would be preferable to single ventricle repair, so this is good news! She also notified me that Dr. Leslie Collins at UAB (pediatric cardiology) would likely want to see me prenatally either for telehealth appointment or in person.
