I hope the one that's covered is the Emend (Aprepitant) Tripack - the one you take the morning of chemo treatment and the 2 days following. That one is rigidly prescribed (do they allow chemo treatment without it?) and is the most outrageously priced - something like $800 for the 3 capsule dose. I remember our co-pay being $83 a pop. The others are optional for nausea symptoms; I know I wasn't too dependent on them. I would think if they require the Emend (medical necessity) you would have some kind of coverage for it - or a generic offering.

This is what I was told some time ago:

Usually when a doctor tells them the drug is needed to sustain life, the insurance company will put it in Tier 3. Meaning you pay a large co-pay but not the entire cost.

Then there is the appeal process where others will review the request. If rejected, the next step is to sue the insurance company and the publicly might encourage them to cooperated more so.
